All Signature Desk request forms are submitted via DocuSign. If you or your office are unable to use DocuSign, please contact the Signature Desk at signaturedesk@austin.utexas.edu with the name of the form you need to access, and a blank PDF copy will be provided.
NEW – The Signature Desk Request Form Requirements Matrix (.xlsx via UTBox) provides requestors with an easy way to look up which signatures are required for each type of Signature Desk request.
